🚨 Form Collection & Final Submission Deadline:
Please collect the physical Graduation Credit Review Form (畢業資格審查表) from the Department Office as soon as possible. After completing your self-check according to the regulations, you must submit the completed form to the Department Office no later than Friday, May 15, 2026.

💻 Online Academic Affairs Query System:
Please log in to the Taipei Tech Portal > Applications > System of Academic Affairs > Students Grades Query System to assist in checking your graduation credit status (detailed requirements remain based on the course standard of your admission year). Cross-check the subjects and grades listed on your physical form to ensure they fulfill all criteria (including common required, professional required, professional elective, and cross-disciplinary elective courses).
✍️ Paper Form Hand-Marking Regulations
🔴 Red Pen Only: Please circle the names of the completed and passed Professional Required Courses on the form.
🔵 Blue Pen Only: Please circle the names of the completed and passed Professional Elective Courses on the form. (If it is a cross-department course, explicitly write "(cross)" right after the subject name).
🏹 Failed & Retaken Courses: If you previously failed a course but later retaken and passed it, please use an arrow to connect the failed course with the retaken passed course on this form.
📝 Midterm Course Withdrawal: For courses that you plan to withdraw from during the midterm this semester, please indicate "WW" (Withdraw) in the grade column of the form in advance.
⚠️ IMPORTANT: Only circle the passed "SUBJECT NAMES". Do NOT under any circumstances alter or circle the number of credits or the grades obtained for the courses!
🔍 Credit Auditing & Exemption Guidelines
- ▪ Credit Transfer & Failed Subjects: If you have applied for credit transfers, the system automatically calculates those credits, displaying "credit transfer" in the remarks. If you passed a failed course by taking a class outside your department but haven't filed for a transfer, please do so at the Academic Affairs Office immediately.
- ▪ Cross-Disciplinary Electives: For cross-disciplinary elective credits, please manually add those credits to your professional elective or cross-department completed credits on the form. No official credit transfer application is required.
- ▪ English Bridge Program Restrictions: Courses passed in the "Professional Business English Bridge Program" before registering an official English test will be counted under "elective credits of cross-specialization." Once an official test score is registered to fulfill the graduation threshold, those same bridge courses CANNOT be double-counted as elective credits.
- ▪ Summary Classification & Signature: Please classify and sum up all your earned common required, professional required, and professional elective credits. Write the total sum in the "Total Earned Credits up to the End of the Fourth Year" column at the bottom. You must hand-sign the form, provide your mobile number, and write your self-check outcome in the Remarks.
- ▪ Minor, Double Major, & Degree Programs: If you are pursuing a Minor, Double Major, Second Major, Program, or Micro-program and have been approved for a study extension, please specify this clearly in the self-check column (e.g., "Minor in XX"). If you need to apply for qualification review, withdrawal, or extension, please submit applications immediately.
💡 Omissions & Grade Discrepancies:
Please check if all credits on the form are accurate. If there are any omissions or errors, please contact the **Department Office** and the **Office of Academic Affairs** in person to clarify. Write the correct subject name, credit number, and grade in the blank space for the current semester, specifying whether it is required or elective. Graduation qualifications are strictly reviewed based on this form; your careful cooperation is highly appreciated!
